The rule in question...
11) The “Minor Leagues” E) Any "minor leaguer" transaction may be nullified and subsequently reversed prior to and after an opposing manager has made a waiver claim if the original owner of the minor leaguer (prior to the waiver claim) unknowingly or accidentally sends the minor leaguer in question to the farm. Each "minor leaguer" will be returned to its owner's main roster and not to its respective farm team.
I've thought a lot about this one, a lot of problems have occurred since its inception but I don't think completely abolishing it is the best solution. I feel like if you actually make a legitimate mistake be it physical or otherwise, if you can notice it and either make note of it on the forum somewhere or contact the Commissioner within 24 hours then it can be forgiven and reversed within this time frame. If you don't notice the mistake and/or make note of it within 24 hours then the player is up for grabs like any other dropped player. This would also protect the Waiver order and assure rosters aren't disturbed or thrown off track.

I very much agree a 24hr window to correct any errors is fair and just.
Actions should have consequences and you need to be sure of how you are managing your franchise.
Otherwise your mistakes will be capitalized on be GMs who are actually paying attention.
